To ensure we can give you the best care and understand your medical history, please bring the below to your initial appointment.
New patients should arrive 30 minutes prior to your appointment start time to allow for registration, insurance collections, and any document review.
For new patients, please plan for an hour for your initial appointment. Follow-up appointments are typically 30-40 minutes.

Bear is our amazing in-office Therapy Dog. His job is to help soothe and support our patients throughout their visit.
Bear is very gentle and very friendly, but if you have an allergy or fear of dogs, simply let us know before arriving. Bear has lots of birds he loves to watch intently, and we’re happy to give you space and him a bird-staring break during your visit.
